There are two versions of “waiver” for your use linked below. The first is to be used for athletes under the age of 18 (Consent and Acknowledgement of Risk) and the second is to be used by any participants aged 18 and above (Release of Liability and Waiver). 

It is expected that any member group participating in the Baseball BC Insurance Program is expected to use these forms and have participants sign off prior to participating.

Member groups not participating in the Baseball BC Insurance program have the option to use these or any created at your end.

Coverage Summary

 


Provincial / National Membership: B.C. Minor Baseball Association, B.C. Little League Baseball, B.C. Babe Ruth Baseball, B.C. Premier Baseball League, B.C. Senior Baseball Association and B.C. Baseball Umpires Association

Associate Membership: as the policy applies to all not listed above under National or Provincial Membership, every player and coach participating in this baseball partnership, excluding the CCBC that affiliate by team, will be assessed a $9.00 Membership fee with free coverage for excecitive & volunteers for their services.  Please note that Membership & the Insurance Policy (see above) commence April 1st and conclude March 31st every year, and therefore, an updated application & member information sheet requesting Associate affiliation, including CCBC teams at $500 per team, must be submitted each & every season (see bottom of page for applications). To avoid challenges or denial to claims, member benifits, etc, accurate applications, payments and registration data for all participants must be provided by the partnering league or association before baseball activities begin and consequently before any claim can be accepted (to submit member info, see Excel sheet at bottom of page with application forms).

* Members wishing to make a claim need to send a brief e-mail,  within a paragraph or so, to info1@baseball.bc.ca explaining the injury, including the members name, team, when & where the injury occurred, and in simplest terms, the cause of & injury itself...   Baseball BC will verify the member by returning the e-mail with a Accident Claim Form which can then be sent to SBC Insurance directly...

* All additional claims that are not due to injury will require a $500-$1000 deductible fee to be accepted & processed.  Email info1@baseball.bc.ca to initiate claims.


E-mail your completed Associate Member form & Participant Member Information excel sheet (links found below) to scottm@baseball.bc.ca, then send a Membership payment cheque to the Baseball BC office or contact Scott MacKenzie (604-586-3315) for more information...
